After the uneasy peace that followed Underworld: Blood Wars, Selene remains in hiding, haunted by the wars she has fought and the cost they have exacted. Hidden somewhere in the ruins of what were once vampire strongholds, she spends her days watching the shifting balance between the vampire covens and the remnant Lycans, hoping that no new threat will force her back into the fight. But the shadows hold more than memories and regrets.

A new faction of vampires—ancient, powerful, and long thought extinct—emerges beneath the old ruins of Eastern Europe. These are “first‑generation” vampires, who carry secrets of the earliest vampire bloodline and believe they hold the key to prophecies long buried. Among them is a “pureblood queen,” whose rise threatens to unravel the fragile peace between species. Selene, drawn out of exile, learns that this emerging threat is also connected to revelations about her own past: a lost daughter and her role in the prophecy that could either save or doom both vampires and Lycans.
Selene reunites with old allies—among them David, the hybrid vampire, and others who once stood at her side—though now loyalties are more uncertain, alliances more fragile. As the pureblood faction gains power, Lycans become involved again, some as adversaries, others as uneasy partners, depending on their interests. Evidence surfaces that those first‑generation vampires may have been experimenting, combining ancient vampire magic with lost lore, possibly even altering the curse that defines both species.

Meanwhile, Selene’s daughter becomes central to the unfolding conflict. Her powers are strong yet volatile, shaped by both vampire and hybrid heritage. The prophecy suggests that she may tip the scale one way or the other: either become a bridge between species or a weapon in a war that threatens to extinguish them all. Selene must protect her, even as she wrestles with guilt, trust issues, and questions of what kind of legacy she wants to leave behind.
The film’s climax builds toward a full‑scale war: the pureblood vampire faction makes aggressive moves to dominate the vampire covens, Lycans rise in response, and Selene must lead a coalition of those still loyal to the old order. Battles rage in underground catacombs, ruined castles, and moonlit forests. Betrayals come from within, particularly among vampire covens where fear, envy, and ambition run deep. Ultimately, Selene confronts the pureblood queen in a desperate duel, seeking to stop her ambition and protect her daughter. Whether she succeeds in preserving any peace—or at least forging a new balance—hinges on sacrifices, both expected and heartbreaking.
